Dr. Svendsen is not currently contracted with any insurances providers. We are happy to provide you with a superbill (medical bill) which you can submit to your insurance company for out of network reimbursement.

Please note that this is not a guarantee that the patient will receive reimbursement for the visit. It is best to contact your insurance provider prior to scheduling an appointment to determine what, if any, amount will be reimbursed.
